| Form volunteers must fill out to work with students on an individual or small group basis during the school year. This form does not need to be completed to volunteer in your child's classroom. This form must be completed every 5 years. | | For use by staff, parents and students. | | Use this form for the school staff to know how your child(ren) should get home should the school close for an emergency. | | These health forms are necessary if your child must be given medication (over-the-counter or prescription) during the school day. Both forms are necessary for prescription medications. | | Scout leaders use this form to reserve room space for scout meetings and activities. Form must be signed by the school principal. | | ALL volunteers must complete this form one time per school year. Forms are also available in the school office next to the volunteer check-in. |
